Autoimmune Encephalitis-A Diagnostic Challenge for the Neurologist

چکیده

Objective Autoimmune encephalitis, is a clinically challenging entity with varied neurological presentations. As autoimmune serology negative in over 50% cases, our objective was to prioritise the use of electroencephalography supported by MRI Brain or PET–CT imaging make definitive diagnosis encephalitis. Background encephalitis consequence an antibody mediated neuronal damage caused cell surface antigens. However, assays can be early stages disease. Hence, we suggest electroencephalogram along brain PET-CT scans avoid diagnostic delays. Design/Methods During span four years [2018–2022] retrospective review case records 50 patients and non-autoimmune were compared. Besides clinical examination, serum cerebrospinal fluid viral assay, electroencephalogram, Magnetic Resonance FDG-PET- CT used confirm diagnosis. Results 60% seronegative 40 % seropositive group while 90% 10% group. Electroencephalography abnormal all cases 100% [50/50] 80% [40/50] In revealed evidence limbic FDG remaining 20%. 10%. Conclusions electroencephalographic abnormalities enabled [p value <0.001]. While electroencephalography, as sample size small further studies are needed these findings.

O 16: Challenge in Diagnosis and Management of Autoimmune Encephalitis

The first description of an autoimmune encephalitis dates back to 1888, when Hermann Oppenheim described a patient with neurological symptoms but no underlying brain pathology. The field of autoimmune encephalitides associated with antibodies targeting cell-surface antigens is rapidly expanding and new antibodies are discovered frequently. Autoimmune encephalitis is a difficult clinical diagnos...

Pseudoachalasia: a diagnostic challenge

Here, we present a case of a 78-year-old man that underwent gastrointestinal endoscopy because of one- monthhistory of dysphagia to liquids and solid foods with accompanying weight loss. On endoscopy, there was distalesophageal stenosis. Multiple biopsies were obtained. Histologic examination of the samples revealed normaltissue. The stenosis was treated by dilatation and abdomino pelvic comput...

Laryngeal Paraganglioma: A Diagnostic Challenge

Paragangliomas are neoplasms of neural crest origin. In the head and neck, they uncommonly involve the larynx. The distinction between paragangliomas and other neuroendocrine tumors can be difficult. Precise diagnosis is important in order to optimize patient treatment. Diagnosis relies mostly on histopathologic examination followed by immunohistochemistry. Here we report a 77-year-old woman wi...

Anti-N-Methyl-D-Aspartate-Receptor Encephalitis, a Challenge for Neurologists or a Chameleon for Psychiatrists: A Case Report

Anti-N-Methyl-D-Aspartate-Receptor (NMDA-R) encephalitis is a new autoimmune disorder, often paraneoplastic in nature, presenting with complex neuropsychiatric symptoms. Diagnosed serologically, this disorder is often responsive to immunosuppressant treatment. We here in report the case of a 56-year-old man with anti NMDA-R encephalitis presenting initially with disorientation and hallucinati...
